Rachel Wood is an Adult-Gerontology Acute Care Nurse Practitioner specializing in advanced heart failure and heart failure with preserved ejection fraction (HFpEF). She works in U-M Department of Disease Management in the Advanced Heart Failure and HFpEF clinics. Her role focuses on providing care to patients in the outpatient clinical setting, with a special emphasis on hospital post-discharge “bridge clinic” appointments aimed at preventing heart failure readmissions. She is also a main contributor to providing specialized heart failure Telemanagement care to heart failure patients in the outpatient setting.

Teaching

Rachel Wood has taught clinical rotations in both traditional and accelerated BSN programs and also functions as an active clinical preceptor for nurse practitioner students in both MSN and DNP programs.
