Rachel Bowers, DNP, MS, AGACNP-BC

Rachel Wood is an Adult-Gerontology Acute Care Nurse Practitioner specializing in advanced heart failure and heart failure with preserved ejection fraction (HFpEF). She works in U-M Department of Disease Management in the Advanced Heart Failure and HFpEF clinics. Her role focuses on providing care to patients in the outpatient clinical setting, with a special emphasis on hospital post-discharge “bridge clinic” appointments aimed at preventing heart failure readmissions.
