Community & Other Practice Initiatives

Overview

The U-M School of Nursing’s practice initiatives are one way the school acts on its commitment to service and provides healthcare for the community on a local to global scale. Staffed and in some cases entirely managed by nursing faculty, these facilities provide high quality direct patient care, addressing a spectrum of healthcare needs as well as population specific concerns.
 

Beaumont Health System

Michigan-based Beaumont Health System has medical centers, community-based sites of care, family and internal medicine practices, nursing centers, home care services, hospice, and hospitals in Oakland, Macomb and Wayne counties. Beaumont also leads the Oakland University William Beaumont School of Medicine in Auburn Hills.

Borgess Health System

Borgess Health System includes hospitals and outpatient clinics in the Kalamazoo area, including the Borgess Heart Institute, Borgess Brain & Spine Institite, Borgess Women's Health, Borgess Vascular Institute, and Borgess Bone & Joint Institute.

Centro de Salud Guamani and Affiliated Local Health Centers

Centro de Salud Guamani is located at the southern end of metropolitan Quito, the capital city of Ecuador, in South America. It provides primary care, obstetric, and emergency services, as well as public health services for a population of more than 85,000 people. Undergraduate students have participated in community health and education activities as part of their community health nursing or global health coursework. They have assisted with in-services for patients, clinical staff, and community outreach workers.

CB Dunbar Hospital in Gbanga, Liberia

Open since 2011, the CB Dunbar Hospital, provides clinical experiences for both undergraduate and graduate students in Gbanga, Liberia (Bong County).  With 55 beds, CB Dunbar aims to become the hospital to which all obstetric and neonatal emergencies in the county are referred. It has specific support from Doctors of the World, both in terms of logistics and human resources.  In 2011, 1,022 babies were born at CB Dunbar Hospital and 93,758 outpatient visits were conducted.

Community Health and Social Services Center (CHASS)

CHASS Center, Inc. is a community-based, not for profit organization formed to develop, promote, and provide comprehensive, accessible and affordable quality primary health care and support services to all residents of the community, with special emphasis on the underserved African-American and Latino populations. CHASS is located in Southwest Detroit and is committed to the overall well-being of the community.

Komfo Anokye Teaching Hospital (KATH), Kumasi, Ghana

Komfo Anokye Teaching Hospital (KATH) is a 1000 bed teaching hospital located in Kumasi, the Regional Capital of Ashanti Region with a total projected population of 3,204,609 (1998). The Obstetrics & Gynecology service has six wards; with three designated as labor and delivery wards. The three labor wards are general, official, and high risk wards. The wards in the service have a bed capacity of 160. The average turnover per bed is 118. The average length of stay in the ward is 4 days with a bed occupancy rate of 132%.  Specialist clinics in asthma in pregnancy, sickle cell and diabetes mellitus commenced in 2008.  There is a busy family planning unit, antenatal care unit, and pediatrics department.

Livingston Pediatrics

Livingston Pediatrics has been providing care to infants, children, and adolescents in Brighton, Michigan and the surrounding area for over twenty-five years. Having a longstanding relationship with the U-M School of Nursing, Livingston Pediatrics is a center where Pediatric and Family Nurse Practitioner students have their clinical rotations and where a faculty member practices.

Manhyia District Hospital, Kumasi, Ghana

Manhyia District Hospital is a Ghana Health Service facility located in the urban area of Kumasi.  Reproductive Health Services include outpatient antenatal care, family planning clinic, in-patient labor and delivery, and adolescent friendly services.  In 2012, over 4000 deliveries and 40,000 thousand outpatient antenatal care visits took place at Manhyia District Hospital.

Packard Health is a non-profit primary care medical practice that has delivered exceptional patient-centered care to thousands of Washtenaw County residents since 1973. Ranked among the top third of primary care practices in Michigan and designated as a Patient-Centered medical Home by Blue Cross Blue Shield of Michigan, Packard Health has an unwavering commitment to ensuring medical care for everyone. Packard Health is located in the Ypsi-Ann Arbor area. 

Phebe Hospital & Bong County Health Team, Suakoko, Bong County, Liberia, West Africa

Located in north central Liberia, Bong County has a population of 328,919 making it the third largest county in Liberia. This county experienced some of the heaviest fighting and destruction during the most recent civil war. There are numerous rural clinics within Bong and two major referral hospitals, including Phebe Hospital, which is capable of providing emergency obstetric and newborn care services (in 2011, there were more than 1,000 deliveries recorded at Phebe). Both undergraduate and graduate nursing students have completed clinical rotations on inpatient units and in outpatient settings at Phebe Hospital.

Regional Alliance for Healthy Schools (RAHS) School-Based Centers

 
RAHS School Based Centers’ mission is to provide high-quality physical, emotional, and preventive health care services to Washtenaw County’s most vulnerable youth in an easily accessible, supportive, and caring environment. RAHS nurse-managed clinics provide care to middle and high school aged adolescents in Washtenaw County schools. See the RAHS website.

Shelter Association of Washtenaw County (SAWC) at the Delonis Health Clinic

The SAWC Health Clinic provides primary care, acute care, and chronic medical care to a variety of vulnerable populations, including underinsured and homeless patients. A feature of the Nurse Practitioner faculty’s practice is their holistic approach in which health promotion, risk reduction, and psychological assessment/treatment are routinely incorporated into overall healthcare practices. Tambon Health Promotion Hospital Dankwian, Chok Chai District, Nakhon Ratchasima, Thailand

Tambon Health Promotion Hospital Dankwian serves 9,026 registered individuals from 1,684 households of 10 villages. This clinic provides basic medical care, school health, family and home care services, and community-based practices. The clinic receives support from Chok Chai District Contracted Unit of Primary Care Collaboration (CUP) that monthly rotates its services to serve individuals with chronic diseases and their complications. The team is composed of a physician, a pharmacist, and a senior nurse. Along with this clinic, other health facilities in this area include 10 communication towers, one village each, and 10 Community Primary Health Centers. There are 10 traditional healers and 171 health volunteers who carry out basic health needs.

Tambon Health Promotion Hospital Ban Kok, Pak Thong Chai District, Nakhon Ratchasima, Thailand

This primary care setting provides comprehensive primary care services including health promotion, disease prevention, medical treatment, and rehabilitation across life span. Care for chronic diseases such as Diabetes and hypertension is offered at this setting through the CUP of Pak Thong Chai District, where a team of a physician, a pharmacist, and a senior nurse of district hospital rotate to the site to provide services every month. The Ban Kok clinic currently serves all 10 villages of the sub-district with a population of about 7,000 individuals. This clinic provides basic medical care, school health, temple health, family and home care services, as well as community-based practices. The clinic works with adult lay health volunteers, school health volunteers, and tradition Thai massage therapy to provide services along with Community Primary Health Centers.

The Taylor Teen Health Center is a school-linked health center that provides high-quality primary healthcare to youth 10-22 years of age and a practice where no one is denied care due to an inability to pay. University of Michigan Adult Medical Short Stay Unit

The Adult Medical Short Stay Unit specializes in the care of short stay medical and procedural patients, most of whom come from the Emergency Department. Care is provided for a range of medical patients that require brief, but intensive monitoring and is individualized to each patient and/or family in order to address and meet their unique needs. University of Michigan Turner Geriatric Clinic

 
Turner Geriatric Clinic is an outpatient program of the UMHS Geriatrics Center that provides comprehensive evaluation and treatment of older adults. The Program provides quality patient care, health and wellness promotion activities, learning programs, and community resource information. In addition to primary care, the Clinic's other specialties include palliative care, neurology, and gero-psychiatry. See their website.
